OF VOLUNTARY TERMINATION OF SUNRISE CARE FACILITY'S PARTICIPATION IN MEDICARE EFFECTIVE SEPTEMBER 8, 2018 Please take Notice that effective September 8, 2018 Down East Community Hospital is voluntarily closing Sunrise Care Facility which is located at 11 Ocean Street, Jonesport, Maine and terminating the Facility's Medicare provider Agreement. Sunrise Care Facility will no longer participate in the Medicare Program (Title XVIII of the Social Security Act) effective September 8, 2018. The agreement between Sunrise Care Facility and the Secretary of Health and Human Services will be terminated on September 8, 2018 in accordance with the provisions of the Social Security Act and the Facility will not be providing any Medicare nursing facility services after that date.
