NOTICE IS HEREBY GIVEN TO Kristina Kulas, mother, whereabouts unknown:
Pursuant to 22 M.R.S. §4001 et seq., the Maine Department of Health & Human Services (hereafter, "DHHS") has petitioned the Court for a Child Protection Order and/or Termination of Parental Rights Order, concerning the child: Cameron D., d/o/b: June 4, 2014, born at Middletown, Connecticut.
The mother is Kristina Kulas, and the father is Michael Kiley or Thomas D'Amico.
DHHS has met the requirements of M.R. Civ. P. Rule 4(g)(1)(A)-(C). Hearing on the pending Petition(s) will be held at Penobscot Judicial Center, 78 Exchange Street, Bangor, Maine, 04401 on April 23, 2018, at 1:00 p.m. Failure to appear at this hearing may result in the issuance of a child protection order, the termination of your parental rights, and/or any other order permissible under 22 M.R.S. §4001 et seq.
